UCSF Trial of Low Dose Naltrexone (LDN) Still Enrolling

I wish the study and it's participants the very best, but I'm not enrolling because I'd have to risk being off LDN temporarily, and I have found through bitter experience that it's a REAL bad idea to get off the stuff; it works perfectly for me as long as I take it; but if I let myself run out, I'm scr*wed!

I have been on for over 2 years and have had extremely stressful situations and have been able to cope and not get any worse.... except when I miss a dose or take it at the wrong time. It works, and I am glad that others who are afraid of off label will get a chance to stay better.

As comments above indicate, its tough to run a study on a readily available drug such as LDN. If people really want to take it, they can already readily obtain it and won't want to risk being in the control arm of the study for any amount of time (a given for a crossover design such as this one).

Most trials offer new substances that aren't available elsewhere to those who are willing to take the risks in order to try it.

Let's hope that this trial gets full enrollment soon!

The folks recommending LDN believe that its efficacy is compromised if you are taking an immunosuppressant such as an interferon. See the 2nd question under the LDN Q&A.

Shouldn't folks not be on the interferons when taking LDN?

I live in cyprus (part of EU) and am wondering how on earth I get a hold of LDN? The american institute of nuerology - which is our ms center here doesnt even know of it let alone prescribe it. I'm british but have lived here in cyprus many yrs.. and need to know how to approach this..?

Για να λάβεις το LDN είναι απαραίτητο μαζί με τα στοιχεία σου και τα στοιχεία της πιστωτικής σου κάρτας να στείλεις με fax και μία συνταγή γιατρού είτε παθολόγου είτε νευρολόγου που θα σου γράψει το LDN.

Το ονοματεπώνυμο του γιατρού και η διεύθυνσή του ιατρείου του ή της κλινικής που εργάζεται θα πρέπει να είναι γραμμένα στα αγγλικά και η συνταγή να περιλαμβάνει τον αριθμό ΤΣΑΥ του γιατρού.

Είναι σημαντικό αν δεν γνωρίζεις εσύ πολύ καλά αγγλικά ώστε να συνεννοηθείς με την φαρμακαποθήκη στην Αμερική στη περίπτωση που υπάρξει κάποιο πρόβλημα με την αποστολή των φαρμάκων, αντί για το κινητό σου να δώσεις το κινητό ενός δικού σου ανθρώπου που θα γνωρίζει όσο το δυνατόν πιο καλά αγγλικά για να μπορέσει να μιλήσει αν του τηλεφωνήσουν για κάποιο λόγο. Να ξέρεις ότι το LDN, δηλαδή η Ναλτρεξόνη έχει διάρκεια ζωής 5 μήνες. Μετά από αυτούς τους 5 μήνες περνάει η ημερομηνία λήξης της. Αυτό σημαίνει ότι οι 300 κάψουλες των 1,5mg θα σου διαρκέσουν ακριβώς για 5 μήνες. Δύο κάψουλες κάθε βράδυ δηλαδή 300/2= 150 ημέρες θεραπείας που ισοδυναμούν ακριβώς με 5 μήνες.

Ο αριθμός fax που θα στείλεις τα στοιχεία σου, δηλαδή το ονοματεπώνυμό σου τη διεύθυνση σου και φυσικά τον αριθμό της πιστωτικής σου κάρτας είναι αυτός.

Όπως είπα και στην αρχή, αν μπορείς μετάφρασε τις οδηγίες για την παραγγελία του LDN ώστε να μπορέσουν να το πάρουν και άλλοι.

Τέλος η συνταγή θα πρέπει να περιλαμβάνει τη διεύθυνσή του ιατρείου ή της κλινικής που εργάζεται και που θα πρέπει να είναι γραμμένα στα αγγλικά.

ΥΓ 1. Είναι σημαντικό η συνταγή να είναι ευανάγνωστη

ΥΓ 2. Η συνηθισμένη δοσολογία είναι 3mg κάθε βράδυ και γι αυτό στη συνταγή αναγράφονται 300 κάψουλες του 1.5mg. Είναι για ακριβώς 5 μήνες, όση δηλαδή και η χρονική διάρκεια μέχρι την ημερομηνία λήξης του φαρμάκου. Μπορείς όμως να παραγγείλεις περισσότερες από 300 κάψουλες, για παράδειγμα 330 ή 350 ώστε να μπορείς μία φορά κάθε λίγες μέρες να παίρνεις 4.5mg. Η προσωπική μου εμπειρία με αυτό το σύστημα δοσολογίας μου έχει δείξει πως έχει ακόμα καλύτερα αποτελέσματα από όταν παίρνω διαρκώς μία σταθερή δόση των 3mg. ΄Καλό θα ήταν όμως τον πρώτο μήνα να παίρνεις τη σταθερή δόση των 3mg μέχρι να είναι έτοιμος ο οργανισμός σου να δεχτεί μία φορά την εβδομάδα μία μικρή αύξηση στη δόση, δηλαδή 4.5mg αντί για 3mg.

The problem with LDN is that many patients report initial improvement, followed by a plateau or loss of effectiveness (as noted at Bethesda conference, April, 2007) for LDN after a few weeks of therapy. This lack of continued effect is explained by counteradaptations to the naltrexone. This problem is solved by the use of a short-acting opiate antagonist, such as naloxone. Doses in the range 10 – 50 mg of naloxone (ie., I.M.) would be given at the onset of therapy. This is gradually increased to 100 or even up to 500 mg daily over several weeks or months. Such high doses are possible due to the short half-life of naloxone. Naltrexone has an effective half-life of > 36 hrs for active metabolites, which would result in continuous blockade with such higher doses. Thus low doses are mandatory with naltrexone. Using naloxone, high doses are possible and preferable, which results in a much greater up regulation of the endorphin system, and thus greater clinical efficacy. Such high doses also have additional benefits, which are too detailed to discuss herein. (Oral formulations are currently being developed by a start-up pharmaceutical corporation.) LDN for me its working wounderful. Also i take L-Carnitine, Roger mac Dougal diet and a lot of training in a gym. Now I have star to swimming and i feel even more better than felt before. For me, this is the first and the only time two years and a half Know Who o Feel better and better. LDN and any medication they mast not be the only way to treat MS.

The BEST way to up regulate the endorphin system, and thus greater clinical efficacy, is training with waits and the swimming. For the last two years I was training with waits in a gym, but know I start also to swimming. The combination of the two (four or three days in a gym and two or three days swimming is probably the best way for endorphins up regulation. Of course with LDN.

Also training with waits and swimming are the best way to treat mitochondrial dysfunction (the ATP production is thw major signal for myelin formation) and much more. Of course, the right diet is a kea component for the best way to ensure the best way to treat ms. Carnitine, Q10, B-complex vitamins, zinc, vitamin C. omega 3, GLA and probably lecithin, all these are working together. (This is an excellent exercise for my English :)
