Survey for people with MS

Hello, my name is Michael Kurland. I am a junior at High Tech High in San Diego, California. I am taking honors biology and part of the requirements for the class is that you complete a science fair project. Since my Mom has Multiple Sclerosis, for my science fair project I chose to do an on-line survey about Multiple Sclerosis. The survey is aimed primarily at food triggers and to some extent is based on my personal observations of my Mom. I would appreciate it if you could help me with this project.
