Resource for tracking your neurologic condition

I just visited PatientsLikeMe and was able to set up a profile in 15 minutes that tracks my MS history, medications, symptoms (you can keep it private).
The site is specific to neurologic condition like MS, ALS, Parkinsons, and for MS they did an impressive job on profiling. And it was intuitive and easy.
